For referring surgeons and their offices

A short route for sending us a patient. We follow your orders and we write back.

  • Send a referral

Use the secure form, fax it to (918) 555-0107 or call the front desk at (918) 555-0106. We confirm that we have it and call the patient to book.

  • What helps us

The patient's name and phone, your contact details, the area of concern, any precautions and your written protocol after surgery.

  • How we follow a protocol

As written, including precautions and limits on motion. If an exam does not fit the protocol, we call your office before changing anything.

  • What your office receives

A written note after the first visit, whenever the plan of care changes and at discharge, sent the way your office prefers.

The notes you receive

Three points at which your office hears from us, whether or not you ask.

  1. 01

Step 1: After the first visit

What we found, what we plan to do and what we need from you, if anything.

  2. 02

Step 2: When the plan changes

What changed and why, and what was agreed with you if the change touches your protocol.

  3. 03

    Step 3: At discharge

How the hand is moving, what the patient does at home and when to expect a call if anything comes up.

Who treats your patient

Three physical therapists, two of whom hold the CHT letters of the Hand Therapy Certification Commission. Tell us the procedure, and the front desk books the therapist who treats it most.

  • Splints and nerve surgery: Lynn Hartman

  • Wrist, elbow and fractures: Darrell Pickett

  • Stiff, swollen and numb hands: Nare Tashjian
